What's inside

A done-for-you, one-page form with space for:

  • Horse name and owner name
  • Session date, last session date and next session date
  • Reason for session: your client's goals or the purpose of the visit (for example, "client states that horse is stiff on the right side")
  • Assessment: what you see with the horse standing or moving (for example, "tight fascia in the right hind end at the trot")
  • Side views of the horse: mark areas of reactivity right on the diagrams
  • Notes: anything else about reactivity or behavior during the session (for example, "multiple yawns while working on the right side glutes")

Is it for you?

Who it's for

Equine massage therapists and bodyworkers who want a clean, professional way to hand clients their session notes, without designing a form from scratch. Print it, take it to your sessions and fill it in as you go.

Why I made this

"Every one of my clients gets written notes and homework after a visit. It's the thing they mention most in their reviews. Good notes show the owner what you found and what to watch for, and they keep you consistent from visit to visit."

Want it with your own branding?

Like what you see, but want your own logo on it or a few different fields? Message me and let's talk.

Ask a question
Can I send it to clients digitally?

Yes. Print it, fill it in by hand at the barn, then scan it with a phone scanner app and email it to your client. I talk through my own process in the session notes podcast episode.

Does it include PEMF?

No. This version is for hands-on bodywork sessions and doesn't include PEMF fields. [CONFIRM WITH JEN: is a PEMF version planned? Stan labels both templates "without PEMF".]

Basic or Advanced: which should I get?

Basic gives you an assessment box and room for notes. Advanced swaps the assessment box for two rating scales (muscle development, and muscle tone and texture), which make it easier to show progress over several visits. They're the same price, so pick the layout that suits how you work.
